What information we collect

We may collect personal and health information including:

  • your name, date of birth and contact details

  • NHI number and referral information

  • medical history and health information relevant to your care

  • information provided through forms, emails or our website

  • payment and billing information

We collect information directly from you where possible. We may also receive information from your GP, other healthcare providers, laboratories, imaging services, pharmacies, ACC or other health agencies involved in your care.

Why we collect your information

We collect and use your information to:

  • provide healthcare and treatment

  • communicate with you about appointments and care

  • coordinate with other healthcare providers involved in your treatment

  • meet our legal and professional obligations

  • manage billing and administration

  • improve our services and website

How your information is stored

Your information is stored securely using trusted practice management and clinical systems. We take reasonable steps to protect your information from loss, unauthorised access, misuse or disclosure.

Some service providers or secure cloud-based systems we use may store data outside New Zealand. Where this occurs, we take steps to ensure your information remains protected in accordance with New Zealand privacy laws.

Sharing your information

We only share your information when necessary for your care, when required by law, or with your consent. This may include sharing information with:

  • your GP or referring clinician

  • other specialists or healthcare providers

  • laboratories and radiology providers

  • ACC or Health New Zealand where required

  • trusted service providers supporting our practice operations

Information collected from other sources (IPP 3A)

Sometimes we receive health information about you from another healthcare provider or agency rather than directly from you. This may include information from:

  • your GP or referring clinician

  • other specialists or hospitals involved in your care

  • HealthOne

  • laboratories, radiology or screening services

  • pharmacies

  • ACC or Health New Zealand

  • family members or support people where appropriate and relevant to your care

Where required, we will take reasonable steps to ensure you are aware that this information has been collected, why it has been collected, and your rights to access and correct it.

About HealthOne

HealthOne is a South Island based secure electronic record that allows registered healthcare providers directly involved in your healthcare, to quickly access information such as your test results, allergies, medications, GP summaries and hospital information. HealthOne strictly adheres to the principles of the Privacy Act 2020 as well as the Rules set out in the Health Information Privacy Code 2020. Access is only possible via an approved highly secure healthcare information network which is regularly audited and tested.  Privacy auditing is used to check that only those directly involved in your care are accessing your information.
